If you are unable to complete the survey online please choose one of the following options:
1. Contact Lonie Sanders at the school district office by phone at 740-622-1901 ext 1001 or by email lelonie.sanders@coshoctoncityschools.com You will need to provide your name, address and the number of students in your household, and we will mail or email the form to you.
2. Stop by the District office at 1207 Cambridge Road between 9:00am and 3:00pm Tuesday, July 28th through Friday, August 7th. There will be copies of the form on the front porch with a submission box for you to drop your form into once it is completed.
3. Print the form off at home and drop it off at the District Office drop box or mail it to the District Office at 1207 Cambridge Road, Coshocton, Ohio 43812
